Participant Registration & Safety Packet

Use only after the club has invited the player to a specific program, tryout or activity.

Important: This packet documents registration and safety acknowledgments. It does not attempt to waive liability for negligence where such a waiver is prohibited or unenforceable under applicable New York law.

Do not email detailed medical records through the public website. If the club needs specific medical information for participation, it should be collected through the club's designated process.

3. Inherent Risk Acknowledgment

I understand that soccer and athletic training involve inherent risks, including running, falls, contact with players or equipment, collisions, strains, sprains, fractures, concussion, heat illness and other injuries. I understand that no sports activity can be made risk-free. I agree to follow applicable safety rules, report injuries and symptoms promptly, and stop participation when instructed by club staff or medical personnel.

This acknowledgment is not intended to release or excuse Saratoga Continentals FC or any other person from liability that cannot lawfully be waived.

4. Emergency Care Authorization

If I cannot be reached promptly during an emergency involving the player, I authorize club personnel to contact 911 or seek reasonable emergency medical assistance. I understand that the club does not provide medical insurance and that medical decisions remain with qualified medical professionals.

5. Concussion & Injury Acknowledgment

I understand that a player with signs or symptoms of a possible concussion or other serious injury should be removed from activity and should not return until appropriate evaluation and clearance requirements are satisfied. I agree to disclose relevant injury symptoms to the club and not pressure a player to continue when safety is in question.

7. Photo / Video Permission

The club should avoid publishing unnecessary personal details about minors, including home address, private contact information and other sensitive details.

8. Accuracy & Authorization

I certify that the information I provided is accurate to the best of my knowledge. If the player is a minor, I certify that I am the parent/legal guardian or otherwise legally authorized to complete this packet.
